2.1 Care Coordination Platform CareAligns provides a communication platform that connects users with healthcare service providers, including but not limited to:

• Hospice care providers

• Home health agencies

• Medical equipment suppliers

• Transportation services

• Skilled nursing facilities

• Therapy providers

2.2 Communication Services Our Services include text messaging coordination through our AI assistant "Caroline" and related communication tools.

2.3 Network Management Users can create care networks including family members and advocates who can coordinate care on their behalf.

3.1 Accurate Information You agree to provide accurate, current, and complete information when using our Services.

3.2 Authorized Use You may only coordinate care for individuals for whom you have explicit authorization to do so.

3.3 Medical Emergencies CareAligns is NOT an emergency service. For medical emergencies, always call 911 or go to the nearest emergency room immediately.

3.4 Provider Relationships CareAligns facilitates introductions to healthcare providers but does not provide medical care, advice, or treatment. All medical decisions are between you and your chosen healthcare providers.

4.1 Information Collection We collect only the information necessary to provide our Services, including contact information and basic care coordination preferences.

4.2 Information Sharing We only share your contact information with healthcare providers you specifically select and only after receiving your explicit consent.

4.3 HIPAA Compliance When applicable, we maintain HIPAA compliance standards for protected health information.

5.1 Explicit Consent Required Before sharing any personal information with healthcare providers, we will request and receive your explicit consent via text message confirmation.

5.2 Withdrawal of Consent You may withdraw consent for information sharing at any time by texting "STOP" or "REVOKE CONSENT."

5.3 Network Authorization By adding family members or advocates to your care network, you authorize them to coordinate care on your behalf using our platform.

6.1 No Medical Advice CareAligns does not provide medical advice, diagnosis, or treatment. We are a communication and coordination platform only.

6.2 Provider Independence Healthcare providers in our network are independent contractors. CareAligns does not control, supervise, or direct their medical practices.

6.3 No Guarantee of Availability While we strive to connect you with available providers, we cannot guarantee that any specific provider will be available or will accept you as a patient.

6.4 Technology Limitations Our Services depend on telecommunications networks, internet connectivity, and other technology systems that may occasionally be unavailable.

TO THE MAXIMUM EXTENT PERMITTED BY LAW, CAREALIGNS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, INCLUDING BUT NOT LIMITED TO LOSS OF PROFITS, DATA, USE, OR OTHER INTANGIBLE LOSSES.

OUR TOTAL LIABILITY FOR ALL CLAIMS RELATED TO THE SERVICES SHALL NOT EXCEED $100 OR THE AMOUNT YOU PAID US IN THE TWELVE MONTHS PRECEDING THE CLAIM, WHICHEVER IS GREATER.

8.1 User Termination You may terminate your use of our Services at any time by texting "DELETE ACCOUNT" or contacting customer support.

8.2 CareAligns Termination We may terminate or suspend your access to our Services for violation of these Terms or for any other reason, with or without notice.

We may modify these Terms at any time. We will notify users of material changes via text message or email. Continued use of our Services after notification constitutes acceptance of the modified Terms.

These Terms are governed by the laws of California without regard to conflict of law principles. Any disputes shall be resolved in the courts of Los Angeles County, California.
